Commissioning an index

In order to provide you with a quotation or a time estimate for completing your index, I would ask you to provide the following information:

When is the index required?

How many pages does the book have?

What is the intended readership group of the book (for example, students, professional practitioners, general etc.) 

Do you have a house style or preferred indexing conventions?

Is there a set fee or would you like a quotation for the work?

What is the preferred length and layout of the index?

You would receive written confirmation of what I understand your requirements to be, and a quotation  for undertaking the work, or agreement to your set fee. Once you had provided me with proofs, I would work on the index and then send this to you in the agreed format.
